We're very excited to announce the arrival of the customizable Release of Information (ROI) portal form!

Release of Information Form

Practice Admins can now enable the Release of Information in the practice Library under Portal Forms.

Form Customization

The ROI can be customized to meet requirements set forth by your state or other governing or credentialing entities.

A Release of Information (ROI) can be sent to the patient for one or more of their contacts, to be completed on the portal.

Completing the ROI in TherapyPortal

The patient or patient’s representative will be able to complete, e-sign, and submit the ROI from within their client portal.

Always Available ROI

The ROI can also be made always available so the patient may complete it at any time, without needing to send a document request. Keep in mind that the patient must select from one of their existing contacts already on file. Therefore, at this time the practice must merge the Patient Contacts Form before the patient can complete the ROI for new contacts.

Viewing ROI Progress

The progress of the client's ROI will be shown with the Document Requests.
