Supervisor Review Overhaul and Much More!

The Supervisor Review feature has been overhauled with new features including: the ability for supervisors to sign notes, the ability to bill as the supervisor, To-Do List integration, and the ability to assign clinicians to supervisors.
Patient contacts: Reversed phones so mobile is first and added birthday
Improved layout of the user home page to make better use of space
Notes display red "Draft" flag when in draft mode
EDI changes to better support payers SX091 (Univera SSA WNY) and 04332 (Network Health)
Updates for iOS 6 compatibility
Numerous other minor tweaks and bug fixes
