What's New: ACE and PHQ-A Outcome Measures

We are excited to announce the newest additions to our Outcome Measures Library, the Adverse Childhood Experiences Questionnaire (ACE) and the Patient Health Questionnaire for Adolescents (PHQ-A). As with all outcome measures, your clients will be able to complete these on the client portal in either English or Spanish. The outcome measure will be scored and ready for your review after the client has submitted it. Completed questionnaires are saved in the client’s Documents tab and can be printed or downloaded as PDFs.

Adverse Childhood Experiences (ACE)

You can send this questionnaire to your clients to understand their experiences with abuse, neglect, and household challenges. 

Patient Health Questionnaire for Adolescents (PHQ-A)

In order to help clinicians better serve their youth and adolescent clients, we have also added the Patient Health Questionnaire for Adolescents (PHQ-A). The PHQ-A helps identify the presence and severity of symptoms of depression.
